Spam by keyword stuffing

What is the difference between content rich in keywords and keyword stuffing. When does content move from the realm of content rich keywords to content spam?
In Keyword Stuffing”, Google defines the practice as, “loading a web page with keywords in an attempt to manipulate a site’s ranking in Google’s search results.”
However, in Webmaster Guidelines Google advises designers to “Think about the words users would type to find your pages, and make sure that your site actually includes those words within it.”
When is keyword stuffing spam and when is keyword stuffing and important part of following Webmaster Guidelines?
Clearly when the web is as large as it is it is not possible to be “found” without ensuring that keywords are targeting your audience. Content is content, but it has to contain the keywords that you are targeting.
Is SEO just about getting away with as much spam like practice as possible without getting pinged?
Is there a formula for keyword density that works in all circumstances. What about derivative words. My keyword is spam but there are those other words like spammer, and spammed which may be also relevant - do they relate to keyword density.
I don’t know - perhaps the only way to find out is to post or create a page and see how it ranks.
Perhaps it’s all a bit “gray hat” after all.
